The following information must be stated in an appeal: the surname, name, pat-ronymic, place of residence, the issue, remarks, suggestions, statements or com-plaints, requests or demands. The applicant must sign it (electronic digital signa-ture is not required), as well as indicate the date and the email address for re-sponse, or other means of communication.
