Filing of Inspection Requests
On March 25, 2015, the Cabinet of Ministers of Ukraine adopted Resolution № 167 On Amendments to the Procedure for Verification of Information on the Application of Prohibitions under P.3 and 4 of Article 1 of the Law of Ukraine On Lustration, which entered into force on April 8, 2015.
Requests for inspections provided for by the Law of Ukraine On Lustration in the form according to Annex 3 (as amended by the Resolution of the Cabinet of Minis-ters of Ukraine of March 25, 2015 № 167) should be sent to the Security Service of Ukraine. The request for an inspection provided for by the Law of Ukraine On Lus-tration shall indicate the email address to which a response may be received about the lack of information on the application of prohibitions.
Copies certified by the signature of the head of the personnel service and sealed shall be attached to the request:
- Statements of the person on carrying out the inspection provided by the Law of Ukraine On Lustration;
- Pages of the passport of a citizen of Ukraine with information about the sur-name, name and patronymic, issuance of the passport and place of registra-tion;
- A document confirming the registration in the State Register of Individuals - Taxpayers (pages of the passport of a citizen of Ukraine - for a person who due to his religious beliefs refuses to accept the registration number of the taxpayer and notified the relevant supervisory authority and has a mark in the passport citizen of Ukraine).
The specified package of documents should be sent to the address: 33 Vo-lodymyrska St., Kyiv, 01601.
In accordance with paragraph 18 of the Procedure for verifying the accuracy of in-formation on the application of prohibitions provided for in parts three and four of Article 1 of the Law of Ukraine On Lustration, approved by the Cabinet of Ministers of Ukraine of October 16, 2014 № 563 and paragraph 10 of the Cabinet of Minis-ters of Ukraine 2015 № 167, requests for persons born after 31.12.1973, the Secu-rity Service of Ukraine are not considered.
